General description & overview of Pyelonephritis (Kidney Infection)

Pyelonephritis is a kidney infection, usually from bacteria that have spread from the bladder. Pyelonephritis is basically an ascending urinary tract infection that has reached the pyelum (pelvis) of the kidney (nephros in Greek). If the infection is severe, the term "urosepsis" is used interchangeably (sepsis being a systemic inflammatory response syndrome due to infection). It requires antibiotics as therapy, and treatment of any underlying causes to prevent recurrence. It is a form of nephritis. It can also be called pyelitis.

Pyelonephritis is very common, with 12-13 cases annually per 10,000 of population in women and 3-4 cases per 10,000 in men. Young women are most likely to be affected, traditionally reflecting sexual activity in that age group. Infants and the elderly are also at increased risk, reflecting anatomical abnormalities and hormonal status.

Symptoms of Pyelonephritis (Kidney Infection)


Symptoms include the following:

  • back, side, and groin pain; basically abdominal pain (radiating to the back on the affected side)

  • tenderness of the bladder area and the side of the involved kidney ("renal angle tenderness")

  • urgent, frequent urination

  • pain or burning during urination, also known as dysuria (painful voiding of urine)

  • fever, rigors (violent shivering while the temperature rises)

  • headache, nausea and vomiting

  • In severe cases, delirium may be present

  • pus and blood in the urine

Natural remedies for Pyelonephritis (Kidney Infection). Severe or recurrent infections may cause permanent kidney damage and lead to chronic kidney disease. In rare cases, infection in the kidney may spread to the bloodstream. Infection of the bloodstream is a serious condition called sepsis. Acute renal failure, a temporary condition in which the kidneys stop working, can also result.

Diagnosis

Diagnosis is made with a urine test to identify bacteria and formations of white blood cells, called casts, shaped like tubes in the kidneys. Formal diagnosis is with culture of the urine; blood cultures may be needed if the source of the infection is initially doubtful. If an infection cannot be easily cured, x rays might be done to look for abnormalities in the kidneys, ureters, and bladder.

If a kidney stone is suspected (e.g. on the basis of characteristic colicky pain, disproportionate amount of blood in the urine), X-rays of the kidneys, ureters and bladder (KUB) may assist in identifying radio-opaque stones.

In patients with recurrent ascending urinary tract infections, it may be necessary to exclude an anatomical abnormality, such as vesicoureteral reflux (urine from the bladder flowing back into the ureter) or polycystic kidney disease. Investigations that are commonly used in this setting are ultrasound of the kidneys or voiding cystourethrography.

Causes of Pyelonephritis (Kidney Infection)


Possible causes of kidney infection include the following:

  • Most cases of "community-acquired" pyelonephritis are due to bowel organisms that enter the urinary tract. Common organisms are E. coli (70-80%) and Enterococcus faecalis infections in the bladder

  • Most cases of pyelonephritis start off as lower urinary tract infections, mainly cystitis and prostatitis.

  • Use of a catheter to drain urine from the bladder (Hospital acquired)

  • Use of a cystoscope to examine the bladder and urethra (Hospital acquired)

  • Surgery on the urinary tract (Hospital acquired)

  • Conditions such as prostate enlargement and kidney stones that prevent the efficient flow of urine from the bladder

  • Constitutional: diabetes mellitus, immunocompromised states

  • Behavioural: change in sexual partner within the last year, spermicide use

  • Positive family history (close family members with frequent urinary tract infections)

  • Follow any dietary recommendations that your doctor advises. Reducing salt intake is often essential as this will minimize fluid retention, swelling, and high blood pressure. It is also advisable to cut down on protein and potassium in your diet, as this will slow down waste build-up in the blood stream.

  • Avoid processed food and anything containing synthetic additives in order to reduce the toxic load on your kidneys. Similarly avoid alcoholic beverages and recreational drugs. 

  • Try to eliminate potential food allergens, including dairy, wheat (gluten), corn, preservatives, and food additives. Your health care provider may want to test for food sensitivities.

  • Eat antioxidant foods, including fruits (such as blueberries, cherries, and tomatoes), and vegetables (such as squash and bell peppers).

  • Eat more high fiber foods, including beans, oats, root vegetables (such as potatoes and yams), and psyllium seed. Bran, a rich source of insoluble fiber, reduces the absorption of calcium, which in turn causes urinary calcium to fall. Risk of forming kidney stones is significantly reduced simply by adding one-half ounce of rice bran per day to the diet. Oat and wheat bran are also good sources of insoluble fiber and are available in natural food stores and supermarkets.

  • Cut down on oxalate-containing foods, such as spinach, rhubarb, beets, nuts, chocolate, black tea, wheat bran, strawberries, and beans. Foods that have medium amounts of oxalate may be eaten in limited amounts. Such foods include grits, grapes, celery, green pepper, red raspberries, fruit cake, strawberries, marmalade and liver.

  • Take probiotic supplement (containing Lactobacillus acidophilus), 5 - 10 billion CFUs (colony forming units) a day, for maintenance of gastrointestinal and immune health. You should refrigerate your probiotic supplements for best results.
